This document ("Terms and Conditions") includes the terms and conditions related to registration at the Mike Tyson Boxing Club ("The Club"). It aims to specify the opening and closing dates for registration, eligibility and subscription restrictions at the Club, procedures for submitting a registration application, screening and the selection process for applicants, general conditions, and the disclaimer and waiver.
The applicant for registration at The Club ("The applicant") acknowledges that they have reviewed the terms and conditions and are fully eligible both legally and procedurally. By submitting a registration application to the Club, The applicant agrees to the following terms and conditions:
1. Opening and Closing Dates:

The registration process at the Club opens on December 1, 2023, and closes on December 26, 2023. No applications will be considered after the specified closing date for registration mentioned above.

2. Eligibility and Subscription Restrictions at the Club:

2.1 The applicant must be at least 18 years old.
2.2 Exception to the provision in paragraph 2.1 above: Individuals below the specified age in paragraph 2.1 may be registered upon submission of a registration request by their legal guardian. This requires obtaining written consent from the legal guardian, and in any case, individuals below the age of 10 cannot be registered at the Club.
2.3 The applicant's age must not exceed 20 years.
2.4 Exception to the provision in paragraph 2.3 above: Individuals above the age specified in paragraph 2.3 may be registered according to conditions set by the Club, following the necessary evaluation. In any case, individuals exceeding the age of 22 cannot be registered at the Club.
2.5 The applicant must be physically, mentally, and medically fit.
2.6 The applicant, or their representative, must be fully eligible both legally and procedurally. Under no circumstances is the applicant allowed to contest otherwise.

3. Procedures for Submitting a Registration Application at the Club:

3.1 Anyone wishing to register at the Club should visit the dedicated website (www.mtbcriyadh.com ) ("Registration Page") to submit their registration applications. They are required to fill in the necessary information and upload the required data through this page.
3.2 The technical committee will screen and evaluate the qualifications of applicants, selecting those who meet the criteria outlined in paragraph 4 of these terms and conditions.
3.3. The applicant, at their own expense, must undergo medical examinations specified by the Club to ensure their medical fitness. The Club retains discretionary authority over the suitability of these medical and fitness examinations. The Club does not cover, under any circumstances, the costs or bear any financial or non-financial responsibilities arising from these examinations.
3.4 The Club will send scheduled appointments to the successful applicants who have passed the medical examinations mentioned in paragraph 3.3 above. This is for conducting fitness and performance tests, and the assessment of these tests falls under the discretionary authority of the Club to evaluate the fitness skills of the candidates.
3.5. The Club will announce the successful candidates who have passed the specified tests and examinations outlined in these terms and conditions. This announcement is made to include them in the Club's team.

4. Screening and Selection Mechanism of Applicants:

4.1 The technical committee performs the screening and evaluation process outlined in paragraph 3 of these terms and conditions based on the experiences and qualifications provided by the applicants on the registration page.
4.2 In the event that the registration application to the Club does not meet the conditions and terms or contains misleading or inaccurate information, the technical committee may reject and exclude it. The committee, at its discretion, may request the applicant to correct the provided information.
4.3 Decisions made by the technical committee regarding registration applications at the Club are final and not subject to any form of appeal.
4.4 The applicant is responsible for the information and data provided. The Club reserves the right to exclude the applicant from the Club at any time if it is found that the information or data provided by the applicant is inaccurate, without prejudice to the Club's right to claim any incurred costs as a result.
4.5 The applicant must submit all supporting documents for the registration application during the performance trials and special tests periods or any other period specified by the Club.
4.6 The applicant must attend the scheduled appointments as per subparagraph 3.4 of paragraph 3 of these terms and conditions. The Club may reschedule those appointments only once. If the applicant fails to attend at the rescheduled time, the applicant will be excluded for non-compliance.
4.7. The applicant is responsible for verifying the dates, times, and locations of the appointments specified in these terms and conditions. The applicant must arrive at least 30 minutes before the scheduled time, and the organizing entity reserves the right to refuse entry to those who arrive late.
4.8 An applicant who has been excluded based on these terms and conditions is not allowed to submit a new registration application to the Club.
4.9 The applicant must adhere to the terms and conditions, laws and regulations applicable in the Kingdom of Saudi Arabia ("the Kingdom"), as well as the internal policies and regulations of the Club. The applicant must also sign any documents deemed necessary by the Club.

5. General Conditions:

5.1 Without prejudice to related regulations and laws, the organizing entity may conduct a security check on the applicant, either directly or through the security apparatus present at the Club. This may involve inspecting the applicant's personal belongings before entering or while inside the Club. The entity also has the right to confiscate any hazardous or prohibited materials according to the regulations and instructions in the Kingdom, following the relevant regulatory procedures.
5.2 Without prejudice to related regulations and laws, the organizing entity, or the security apparatus at the Club, may confiscate any material deemed dangerous or potentially harmful to the location, individuals, or site integrity. This action will be taken in accordance with the regulatory procedures in place.
5.3 The applicant is not allowed to enter the Club's arena without the approval of the coach or the gym supervisor.
5.4. The applicant is obligated not to bring accompanying individuals to the Club unless the presence of the applicant's legal guardian is required.
5.5 The organizing entity, at its discretion, has the authority to exclude anyone who violates the regulations, laws, and instructions applicable in the Kingdom, including the Club's code of conduct.
5.6 In case the organizing entity cancels or changes the specified dates in these terms and conditions, it must inform the applicant of such changes and schedule a new appointment accordingly.
5.7 The Club is not responsible for any damage, loss, or theft that may occur to personal belongings of The applicant stored only in the Club's lockers, except during official Club operating hours.
5.8. Entry of any fixed or mobile devices designed to capture still or moving images, excluding mobile phones, into the Club is prohibited without prior approval from the Club.
5.9 The applicant is obligated to exercise sufficient care for the devices, tools, and materials owned by the Club. In the event that the applicant causes damage, loss, or destruction to any devices, tools, materials, or any Club property due to their error or negligence, the applicant is responsible, and the Club has the right to claim the costs incurred.
5.10 The organizing entity is not responsible for bodily injuries, illnesses, deaths, or the loss or damage of personal belongings of visitors.
5.11 Smoking is prohibited inside the Club and its facilities. Smoking, as mentioned in this paragraph, includes the use of tobacco and its derivatives, including electronic cigarettes, regular cigarettes, cigars, pipes, snuff, and any product containing tobacco components, whether through electronic cigarettes, regular cigarettes, cigars, hookahs, chewing, storage, or any other method.
5.12 Bringing or accompanying animals inside the Club and its facilities is not allowed.
5.13 The organizing entity or the Club, under certain circumstances, is not responsible for bodily injuries, illnesses, deaths, or the loss or damage of personal belongings of Club visitors.
5.14. The applicant is required to bring all necessary equipment for engaging in sports activities, and they are not allowed to engage in any sports activity at the Club without wearing the appropriate clothing and equipment.
5.15 The applicant is obligated to follow the instructions issued by their coach or the Club.
5.16 The applicant acknowledges that they have agreed to these terms and conditions, and this agreement serves as all necessary consents required under the regulations and laws in effect in the Kingdom, including the Personal Data Protection System.

6. Disclaimer and Waiver ("Waiver")

6.1 The applicant (referred to in the waiver as the "Assignee") acknowledges, under these terms and conditions, to release and indemnify the Club, its owners, affiliates, employees, volunteers, and others (referred to in the waiver as the "Waiver Recipient") from any liability regarding the Assignee's participation with the Waiver Recipient.
6.2. The Assignee acknowledges, to the maximum extent permitted by the regulations and laws in effect in the Kingdom, to fully release and discharge the Waiver Recipient from any and all responsibilities, claims, or demands arising from any injury, loss, or damage—whether physical, material, or moral—incurred by them or their properties. This includes any deformity, paralysis, or death that may occur during preparation for or participation in any activities or services provided by the Waiver Recipient, whether resulting from intentional or unintentional negligence, whether due to the actions of the Waiver Recipient or an act of a third party, either directly or indirectly.
6.3 The Assignee acknowledges that if they notice any dangers while at the premises of the Waiver Recipient or participating in the activities or services provided by the Waiver Recipient, they will immediately cease the activity and inform the responsible party at the Waiver Recipient.
6.4 The Assignee acknowledges that they are fully aware and agree that their participation with the Waiver Recipient involves known and unknown risks and hazards that cannot be anticipated. They recognize that these risks may include, but are not limited to: equipment malfunctions or failure, defects or negligence in the design or manufacture of equipment, improper operation or use, collisions or impacts with other participants, the potential for being pushed, vibrated, recoiled, thrown forward or backward, slipping or wet equipment.
6.5. The Assignee acknowledges that they assume all risks associated with their participation with the Waiver Recipient, including, but not limited to, all risks outlined in the terms and conditions. They acknowledge that these risks may include physical, mental, psychological, and personal injury, or damage to property, deformity, paralysis, or death to themselves or others. The Assignee accepts responsibility for such risks, whether mentioned or not in these terms and conditions.
6.6. The Assignee acknowledges and affirms that they are in good physical and mental health, and have not previously suffered from any injury, medical condition, illness, ailment, or disorder that might make their participation with the Waiver Recipient more hazardous, either for themselves, others, or increase the likelihood of an accident resulting in injury, medical condition, illness, ailment, recurrent weakness, deformity, dislocation in their body, recurring loss of consciousness, dizziness, brain disease, nervous system disease, high blood pressure, lung or heart disease, or disorders of other glands, or any condition requiring regular medication use. They also confirm that they are not afflicted with physical or mental illnesses, disabilities, chronic diseases, or any type of injury, and are not undergoing treatment for any physical or mental illness or chronic disease of any kind.

شروط وأحكام التسجيل بنادي مايك تايسون للملاكمة

تتضمن هذه الوثيقة ("الشروط والأحكام") الشروط والأحكام المتعلقة بالتسجيل في نادي مايك تايسون للملاكمة ("النادي")، كما تهدف إلى بيان تاريخ افتتاح التسجيل وإغلاقه، وأهلية وقيود الاشتراك في النادي، وإجراءات تقديم طلب التسجيل في النادي، والفرز وآلية اختيار المتقدمين، والشروط العامة ، وإخلاء المسؤولية والتنازل.
يقر مقدم طلب التسجيل في النادي ("المتقدم")، بأنه اطلع على الشروط والأحكام وهو بكامل أهليته المعتبرة شرعًا ونظامًا، وأنه بمجرد قيامه بتقديم طلب التسجيل في النادي؛ فإنه يوافق على الشروط والأحكام الآتية:
1. تاريخ افتتاح التسجيل وإغلاقه:
يفتح باب استقبال طلبات التسجيل في النادي بتاريخ 1/12/2023م، ويُغلق باب التسجيل فيه بتاريخ 26/12/2023م؛ ولن يتم النظر في أي طلب بعد تاريخ إغلاق التسجيل المشار إليه أعلاه.

2. أهلية و قيود الاشتراك في النادي:

(2.1) لا يجوز أن يكون عمر المتقدم أقل من (18) ثمانية عشر سنة.
(2.2) يجوز -استثناءً من حكم الفقرة (2,1) أعلاه- أن يتم تسجيل منهم أقل من العمر المحدد في هذه الفقرة (2,1) أعلاه؛ وذلك بناءً على طلب تسجيل يقدمه ولي أمره وبعد أخذ موافقه خطية من ولي أمره على ذلك، وفي جميع الأحوال لا يجوز تسجيل منهم أقل من (10) عشرة سنوات في النادي.
(2.3) لا يجوز أن يتجاوز عمر المتقدم (20) عشرون سنة.
(2.4) يجوز -استثناءً من حكم الفقرة (2,3) أعلاه- أن يتم تسجيل منهم أكبر من العمر المحدد في هذه الفقرة (2,3) أعلاه؛ وذلك وفق الشروط التي يحددها النادي وبعد إجراء التقييم اللازم له، وفي جميع الأحوال لا يجوز تسجيل من يتجاوز عمره (22) اثنين وعشرون سنة في النادي.
(2.5) يجب أن يكون المتقدم لائقًا صحيًا وبدنيًا وعقليًا.
(2.6) يجب أن يكون المتقدم -أو من ينيبه- بكامل أهليته المعتبرة شرعًا ونظامًا؛ ولا يجوز له -بأي حالً من الأحوال-الاحتجاج بخلاف ذلك.

3. إجراءات تقديم طلب التسجيل في النادي:
تكون إجراءات تقديم طلب التسجيل في النادي، وفقًا للآتي:

(3.1) على كل من يرغب بالتسجيل في النادي الدخول على الرابط الإلكتروني (www.mtbcriyadh.com ) ("صفحة التسجيل") المخصص؛ لاستقبال طلبات التسجيل في النادي وتعبئة المعلومات والبيانات المطلوبة، ورفعها من خلاله.
(3.2) تقوم اللجنة الفنية بفرز وتقييم مؤهلات المتقدمين، وتختار المجتازين منهم وفقًا للفقرة رقم (4) من هذه الشروط والأحكام.
(3.3) يقوم المتقدم -على نفقته الخاصة- بإجراء الفحوصات الطبية التي يحددها النادي؛ للتأكد من لياقته الطبية، وتخضع تلك الفحوصات الطبية واللياقية إلى سلطة النادي التقديرية في مدى مناسبتها؛ ولا يُغطي النادي -بأي حالً من الأحوال- تلك الفحوصات أو يتحمل أي تكاليف مالية أو غير مالية تنشأ عنها.
(3.4) يقوم النادي بإرسال مواعيد مجدولة للمتقدمين المشرحين والمجتازين للفحوصات الطبية المشار إليها في الفقرة (3,3) أعلاه؛ لإجراء اختبار اللياقة والأداء، ويخضع ذلك الاختبار لسلطة النادي التقديرية في مدى مناسبة المهارات اللياقية للمرشحين والمجتازين.
(3.5) يقوم النادي بالإعلان عن المتقدمين المجتازين للاختبارات والفحوصات المنصوص عليها في هذه الشروط والإحكام؛ وذلك لضمهم إلى فريق النادي.

4.الفرز  وآلية اختيار المتقدمين:

(4.1) تقوم اللجنة الفنية بعملية الفرز والتقييم المنصوص عليها في الفقرة رقم (3) من هذه الشروط والأحكام؛ بناءً على الخبرات والمؤهلات المقدمة المتقدمين في صفحة التسجيل.
(4.2) في حال كان طلب التسجيل في النادي غير مستوفي للشروط والأحكام، أو كان يحتوي على معلومات أو بيانات مضللة أو غير صحيحة؛ فإنه يجوز للجنة الفنية رفضه واستبعاده؛ وللجنة -وفقًا لتقديرها- أن تطلب من المتقدم أن يصحح معلوماته المقدمة منه.
(4.3) تكون القرارات الصادرة عن اللجنة الفنية بشأن طلبات التسجيل في النادي قرارات نهائية لا يجوز التظلم عليها بأي طريقةً كانت.
(4.4) يكون المتقدم مسؤولًا عن المعلومات والبيانات التي قدمها؛ ويجوز للنادي -في أي وقت- استبعاد المتقدم من النادي؛ إذا تبيّن له أن المعلومات أو البيانات التي قدمها المتقدم غير صحيحة؛ وذلك دون الإخلال بحق النادي في المطالبة بالمبالغ التي تكبدها جراء ذلك.
(4.5)ي جب على المتقدم أن يُقدم جميع الوثائق الداعمة لطلب التسجيل؛ وذلك خلال فترات تجارب الأداء والاختبارات الخاصة أو في أي فترة أخرى يحددها النادي.
(4.6) يجب على المتقدم الحضور في المواعيد المجدولة حسب الفقرة الفرعية رقم (3,4) من الفقرة رقم (3) من الشروط والأحكام، ويجوز للنادي إعادة جدولة تلك المواعيد -لمرة واحدة فقط-؛ وفي حال لم يحضر المتقدم في تلك المواعيد؛ فإن المتقدم يُستبعد لعدم التزامه بذلك.
(4.7) يكون المتقدم مسؤولًا عن عملية التحقق من تاريخ ووقت ومكان المواعيد المنصوص عليها في هذه الشروط والأحكام، كما يجب على المتقدم الحضور قبل (30) ثلاثين دقيقة على الأقل من الموعد المحدد، ويحق للجهة المنظمة أن ترفض دخول المتأخرين عن ذلك.
(4.8) لا يجوز للمتقدم المستبعد بناءً على هذه الشروط والأحكام؛ أن يقدم طلبًا جديدًا للتسجيل في النادي.
(4.9) على المتقدم أن يلتزم بالشروط والأحكام، والأنظمة واللوائح المعمول بها في المملكة العربية السعودية ("المملكة")، والسياسات واللوائح الداخلية للنادي، كما يجب عليه توقيع أي وثائق يراها النادي ضرورية.

5.الشروط العامة:

(5.1) مع عدم الإخلال بالأنظمة واللوائح ذات العلاقة؛ فإنه يجوز للجهة المنظمة أن تجري الفحص الأمني للمتقدم؛ وذلك من خلالها أو من خلال الجهاز الأمني المتواجد في مقر النادي؛ وذلك بفحص المقتنيات الشخصية للمتقدم قبل دخوله للنادي أو أثناء تواجده فيه؛ ولها كذلك أن تتحفظ على أي مواد خطرة أو محظورة بموجب الأنظمة واللوائح والتعليمات السارية في المملكة؛ وذلك وفق الإجراءات النظامية المتبعة بهذا الشأن.
(5.2) مع عدم الإخلال بالأنظمة واللوائح ذات العلاقة؛ فإنه يجوز للجهة المنظمة أن تتحفظ على أي مادة -ترى الجهة المنظمة أو الجهاز الأمني المتواجد في مقر النادي- أنها خطرة أو يمكن أن تُستخدم؛ لإلحاق الضرر بالمكان أو الأشخاص أو لإفساد الموقع.
(5.3) لا يجوز للمتقدم دخول حلبة النادي؛ إلا بعد الحصول على موافقة المدرب أو مشرف الصالة.
(5.4) يلتزم المتقدم بعدم اصطحاب المرافقين إلى النادي؛ إلا في حال تطلب وجود ولي أمر المتقدم.
(5.5) للجهة المنظمة -وفقًا لتقديرها- بأن تقوم باستبعاد كل من يخالف الأنظمة واللوائح والتعليمات السارية في المملكة؛ بما فيها لائحة المحافظة على الذوق العام من النادي.
(5.6) في حال ألغت الجهة المنظمة أو غيرت المواعيد المحددة في هذه الشروط والأحكام؛ فإن عليها إبلاغ المتقدم بذلك، وأن تقوم بتحديد موعد جديد عوضًا عنه.
(5.7) لا يتحمل النادي أي مسؤولية عن أي ضرر أو تلف أو فقد يحدث للممتلكات الشخصية للمتقدم والتي تكون في خزائن النادي فقط؛ إلا في أوقات عمل النادي الرسمية.
(5.8) يُمنع دخول أي أجهزة ثابتة أو متحركة، مُعدة لالتقاط الصور الثابتة أو المتحركة -باستثناء الهواتف النقالة- للنادي؛ إلا بعد الحصول على موافقة النادي المسبقة.
(5.9) يلتزم المتقدم بأن يبذل العناية الكافية بالأجهزة والأدوات والخامات المملوكة للنادي؛ وفي حال تسبب المتقدم في تلف أو فقدان أو تدمير أجهزة أو أدوات أو خامات أو أي من ممتلكات النادي وكان ذلك بسبب خطأ المتقدم أو إهماله؛ فيكون المتقدم مسؤولاً عن ذلك، وللنادي الحق في المطالبة بالمبالغ التي تكبدها جراء ذلك.
(5.10) لا تتحمل الجهة المنظمة مسؤولية الإصابات الجسدية أو الأمراض أو وفاة أي من الزوار أو فقدان أو تضرر مقتنياتهم الشخصية.
(5.11) يُحظر التدخين داخل النادي ومرافقه؛ ويقصد بالتدخين -المذكور في هذه الفقرة- تعاطي التبغ ومشتقاته، ويشمل ذلك السجائر الإلكترونية والسجائر العادية والسيجار والجراك والتنباك والمعسل وأي منتج يدخل التبغ في مكوناته، سواء تم ذلك عن طريق السجائر الإلكترونية أو السجائر العادية أو السيجار أو الغليون أو النشوق أو الشيشة أو المضغ أو التخزين أو أي طريقة أخرى.
(5.12) يُحظر إحضار أو اصطحاب الحيوانات داخل النادي ومرافقه.
(5.13) لا تتحمل الجهة المنظمة أو النادي -بحسب الأحوال- أي مسؤولية عن الإصابات الجسدية أو الأمراض أو الوفيات أو فقدان أو تضرر المقتنيات الشخصية؛ لأيً من زوار النادي.
(5.14) يلتزم المتقدم بإحضار كافة لوازم ممارسة النشاط الرياضي، ولا يجوز له ممارسة أي نشاط رياضي في النادي؛ إلا بعد ارتداء الملابس والمعدات المخصصة لذلك.
(5.15) يلتزم المتقدم بالتعليمات التي تصدر من مدربه أو النادي.
(5.16) يقر المتقدم بأنه وافق على هذه الشروط والأحكام وتكون هذه الموافقة بمثابة جميع الموافقات المطلوب أخذها بموجب الأنظمة واللوائح والتعليمات السارية في المملكة؛ بما في ذلك نظام حماية البيانات الشخصية.

6.إخلاء المسؤولية والتنازل ("التنازل"):

(6.1) يقر المتقدم (ويشار إليه في التنازل بـ "بالمُتنازل") بأن يخلي -بموجب هذه الشروط والأحكام- ويعفي النادي وملاكه وتابعيه من مسؤولين وموظفين ومتطوعين وغيرهم (ويشار إليهم في التنازل بـ "المتنازل له") من أي مسؤولية؛ فيما يتعلق بمشاركة المُتنازل لدى المتنازل له.
(6.2) يقر المُتنازل بأن يخلي ويعفي -إلى الحد الأقصى المسموح به بموجب الأنظمة واللوائح والتعليمات السارية في المملكة- عن المتنازل له بشكلً كامل عن جميع المسؤوليات أو الدعاوى، أو المطالبات التي تنشأ عن أي ضرر أو خسارة أو إصابة تقع عليه -سواءً جسدية أو مادية أو معنوية-، أو تقع على ممتلكاته، بما في ذلك عند تعرضَه للتشوه أو الشلل أو الوفاة أثناء التحضير للمشاركة أو خلال المشاركة في أي من الأنشطة أو الخدمات التي يقدمها المتنازل له سواء كانت ناتجة عن إهمال بقصد أو بغير قصد سواءً كانت بسبب فعل مرتكب أو فعل مُغفل عنه، مباشر أو غير مباشر.
(6.3) يقر المُتنازل بأنه في حال لاحظَ أي أخطار أثناء تواجده في مقر المتنازل له أو مشاركته في الأنشطة أو الخدمات التي يقدمها المتنازل له؛ فإنه سوف يقوم بالتوقف عن ممارسة النشاط وسُيبلغ المسؤول المعني لدى المتنازل له فوراً.
(6.4) يقر المُتنازل بأنه يعلم ويعي تماماً ويوافق بشكلً كامل على أن اشتراكه لدى المتنازل له ينطوي على مخاطر وأخطار معلومة وغير معلومة ولا يمكن توقعها. كما يقر بأن هذه الأخطار تشمل، على سبيل المثال لا الحصر: أعطال المعدات أو توقفها عن العمل، عيب أو إهمال بتصميم أو تصنيع المعدات، التشغيل أو الاستخدام بشكل غير سليم أو بإهمال، السقوط أو الاصطدام بالمشاركين الآخرين، احتمال التعرض للدفع أو الاهتزاز أو الارتداد أو الإلقاء إلى الأمام أو الخلف أو الاهتزاز أو فقد التوازن أو عدم التماسك، وجود معدات زلقة أو مبللة.
(6.5) يقر المتنازل بأنه يتحمل جميع الأخطار المرتبطة باشتراكه لدى المتنازل له، بما في ذلك على سبيل المثال لا الحصر، جميع الأخطار المنصوص عليها في الشروط والأحكام، ويقر بأن هذه الأخطار قد تشمل الإصابة الجسدية والمعنوية والنفسية والشخصية، أو الأضرار التي تلحق بالممتلكات أو التشوه أو الشلل أو الوفاة له أو لغيره، ويقر بتحمل مثل هذه الأخطار وتحمل المسؤولية كاملة فيما يتعلق بهذه الأخطار، سواء كانت مذكورة أو غير مذكورة في هذه الشروط والأحكام.
(6.6) يقر المُتنازل ويؤكد بأنه بحالة صحية جسدية ومعنوية جيدة ولائق بدنيًا، ولم يسبق له أن عانى أو يعاني من أي إصابة أو حالة طبية أو مرض أو علة أو اضطراب ممكن أن يجعل اشتراكه لدى المتنازل له أكثر خطورة سواءً بالنسبة له أو للآخرين أو يزيد من احتمالية وقوعه في حادث قد يؤدي إلى أي إصابة أو حالة طبية أو مرض أو علة أو ضعف متكرر أو تشوه أو خلع في جسمه أو فقدان وعي متكرر أو دوخة أو مرض في الدماغ أو الجهاز العصبي أو ارتفاع ضغط الدم أو مرض في الرئتين أو القلب أو اضطراب غدد أخرى أو أي حالة تتطلب استخدام الأدوية بانتظام. كما يؤكد بأنه ليس مصابًا بأمراض جسدية أو نفسية أو إعاقة أو مرض مزمن أو أي إصابة من أي نوع، ولا يخضع للعلاج من مرض جسدي أو نفسي أو مرض مزمن من أي نوع.
